Transitioning from Child Services to Adult Services

The Transitions Service provides support to young people moving from Child Services into Adult Services.  The aim is to provide a seamless plan for each individual and their families/carers.  This is a high priority for partner organisations and work around the All Age Disability Service.

For Children and Young People who are supported by the Children with Disability Team (CWD), CWD staff carry out the Care Act Assessment as the starting point for understanding needs and outcomes for these young people.  CWD work closely with Adult Social Care (ASC) in developing a support plan that supports each individual in education and employment, with a key focus on promoting independence.  The teams work closely with health colleagues and commissioners to consider all needs of the young people, and to ensure there are a range of appropriate services and support to help them achieve their goals and aspirations into adulthood.
